‘If cough persists…’: Govt discourages steroid use for Covid treatment

In its revised guidelines, the government said there was no evidence that not requiring supplemental oxygen or continued use of injectable steroids after hospital discharge would benefit Covid-19 patients. Clinical guidelines issued jointly by AIIMS, Indian Council of Medical Research (ICMR) and Covid-19 National Task Force and Joint Monitoring Group warn that anti-inflammatory treatments may lead to the risk of secondary infections such as invasive mucormycosis or ‘black fungus’ ‘.

Under anti-inflammatory or immunomodulatory therapy, the guidelines say, “in Covid-19 patients,” injection of methylprednisolone 0.5 to 1 mg/kg in two divided doses or an equivalent dose of dexamethasone, usually administered 5 to 10 days”. If the patient’s condition stabilizes and/or improves, it can be administered orally.

Adult Covid-19 patients were classified into mild, moderate and severe disease based on upper respiratory symptoms and dyspnea. The government-recommended treatment does not include drugs such as ivermectin, favipiravir and doxycycline. Antiviral drugs such as molnupiravir and monoclonal antibodies are also not included in the guidelines.
